Selecta contains a medicine called bisoprolol, which belongs to a group of medicines called beta blockers. These medicines work by affecting the body's response to certain nerve impulses, especially in the heart. As a result, the heart beats less and makes the heart more efficient at pumping blood around the body. Heart failure occurs when the heart muscle becomes weak and is unable to pump enough blood to meet the body's needs. Selecta is used in combination with other medicines suitable for this condition, such as angiotensin-converting enzyme (ACE) inhibitors , diuretics and cardiac glycosides. How to use Regular monitoring by your doctor is essential when starting treatment and when increasing the dose. ™ Treatment with Selecta Take the tablet with a little water in the morning, with or without food. Do not crush or chew the tablet. Treatment with Bisoprolol Fumarate Tablet should be started at a low dose and then gradually increased. Your doctor will decide how to increase the dose, and this is usually done in the following way: • 1.25 mg bisoprolol fumarate once daily for one week • 2.5 mg bisoprolol fumarate once daily for one week • 3.75 mg bisoprolol fumarate once daily for one week • 5 mg bisoprolol fumarate once daily for four weeks • 7.5 mg bisoprolol fumarate once daily for four weeks • 10 mg bisoprolol fumarate once daily for maintenance treatment. The maximum recommended daily dose of bisoprolol fumarate is 10 mg Warnings and precautions • You have severe asthma or chronic lung disease. • You have a severe problem with blood circulation in your extremities (such as Raynaud's syndrome), which may cause your fingers and toes to cramp or turn pale or blue. • You have an untreated malignant tumor, a rare type of adrenal gland tumor. • You have metabolic acidosis, a condition when there is too much acid in your blood. • Acute heart failure or heart failure that suddenly becomes worse and/or that may require hospitalization • Slow heart rate • Severe low blood pressure • A certain heart condition that causes a very slow heart rate or an irregular heartbeat. • Cardiogenic shock, a serious, acute heart condition that causes low blood pressure and circulatory failure. Side Effects To prevent serious side effects, talk to your doctor right away if any side effects are severe, happen suddenly, or get worse quickly. Slow heart rate Worsening heart failure Slow or irregular heartbeat • Tiredness, feeling tired, dizziness, headache. • Cold or tingling sensation in the hands or feet. • Low blood pressure. • Stomach or intestinal disorders causing nausea, vomiting, diarrhea, or constipation. • Sleep disturbances. • Breathing problems in people with asthma or chronic lung disease. • Depression. • Muscle weakness and muscle cramps. • Hearing problems. • Allergic runny nose. • Hepatitis causing yellowing of the skin or the whites of the eyes. • Allergic-like reactions such as itching, rash and feeling hot. • Decreased sexual performance (impotence). • Nightmares and hallucinations. • Loss of consciousness. Storage Information Keep this medicine out of the sight and reach of children. Do not use this medicine after the expiry date which is stated on the carton. The expiry date refers to the last day of that month. Do not store above 30°C. Keep container tightly closed.Do not use if you notice any damage to the outer packaging Drug Interactions • Blood pressure control medications or heart problems medications (such as amiodarone, amlodipine, clonidine, digitalis glycosides, diltiazem, disopyramide, felodipine, flecainide, lidocaine, methyldopa, moxonidine, phenytoin, propafenone, quinidine, rilmenidine) • Antidepressants such as imipramine, amitriptyline, moclobemide • Medications to treat mental illness such as phenothiazines such as levomepromazine • Medicines used for anesthesia during surgery (see also “Take care with bisoprolol tablets”) • Medicines used to treat epilepsy, eg. barbiturates such as phenobarbital • Some pain relievers (such as acetylsalicylic acid, diclofenac, indomethacin, ibuprofen, and naproxen) • Asthma medications or medications used to treat nasal congestion • Medicines used for certain eye disorders such as glaucoma (increased pressure in the eye) or used to dilate the pupil • Some medications for the treatment of clinical shock (such as adrenaline, dobutamine, noradrenaline) • Mefloquine, an antimalarial drug • All of these medications, in addition to bisoprolol, may affect blood pressure and/or heart function. • Rifampicin for the treatment of infections • Medicines to treat severe headaches or migraines (ergotamine derivatives). Ingredients • The active ingredient in Selecta tablets is bisoprolol fumarate. Specifications Selecta 10 mg tablets are white to off-white, round, biconvex, film-coated tablets, engraved on one side with `JP103` and on the other side with a score line.
